FREE WORKSHEET

The Fading Account Worksheet

Find dentist accounts whose case volume may be starting to fade before they disappear completely.

FILL IT IN HERE

Find your fading accounts now

Enter each practice or doctor with the cases they sent in the latest full month and in the six full months before it. The worksheet flags the biggest drops and builds your call list as you type.

Nothing you type leaves your browser. Nothing is saved or sent to Lecron. Use practice or doctor names only: no patient names, scans, clinical notes, or case details that identify a patient.

1. Set your rules

Start with a 30% drop and two cases. Adjust the rules to your lab. They are working rules, not research benchmarks.

2. Enter your accounts

Use one name per account. Count each held, late or remade case once, in the 90 days ending at the review month-end, and enter zero only after checking. Missing history is not zero: a new account shows "No baseline" and you judge it by hand.

Practice or doctor Cases, earlier six months (total) Cases, latest month Held, late or remade in 90 days (optional) Other signal (optional) Result Remove

3. Your call list

A flag is a reason to ask, not proof that a dentist is leaving. Check holidays and clinic closures first.

Enter at least one account with both case counts to build the list.

The conversation

Open

Dr. [Name], this is [Name] at [Lab]. I noticed we have been doing less work together lately. Has anything about working with us made your day harder?

If business is slow

Thank you for telling me. Is there one thing about working with us that you would change?

Agree on the next step

Thank you for telling me. Here is what I will do: [action]. I will contact you on [date] to check whether it helped.

Listen without defending. Record the answer accurately. Do not promise a fix you cannot deliver. Keep the agreed follow-up date.

What it helps you do

How to use it

  1. Pull a case report from your lab system, grouped by practice or doctor, for the latest full month and the six full months before it.
  2. Enter each account in the worksheet above, with any holds, late cases or remakes on their recent work. The call list builds itself as you type.
  3. Use the conversation guide under the call list to decide who to contact first and what to ask. The printable version carries the same steps for a paper review.
